• The application form below completed in English ;
• The applicant's logo in high definition (PNG or JPEG) send at evenement(@)chambre.cz ;
• Optional: any documents that may assist the jury in its analysis of the nominee's performance and/or activity or project. These documents must be written in English and sent at evenement(@)chambre.cz.
